Submitting the Form:

If you have an email client such as Outlook, Eudora, Spicebird, Thunderbird, etc., you can submit the form using the “Submit by Email” button on the form. It will generate an email in your email client to send to us.

If you are using an internet Email service without any email client, such as Hotmail, Gmail, Yahoo Mail, etc. Then you will need to save the form on your system first and then manually attach it to an email. Note: If you are using Adobe Acrobat Reader, you may not be able to save the form with your responses. In this case you will need to print the form and send us the hard copy.
